Visa Requirements

Tourist / Visitor Application form and Additional forms completely filled and signed by the applicant.
Two recent passport-size photographs (3.5cm x 4.5cm) in color with good resemblance, 60-80% face coverage. Photograph should not be more than 3 months old.
The Passport should be valid for at least three months (any other type of travel document should be valid for at least six months), beyond the intended duration of stay in the Schengen territory and with at least two blank pages to affix the visa. Passports valid for more than 10 years will not be accepted as the main required travel document.
All the Old Passports.
Typed Cover letter on Indian’s Company letter head if self Employed along with Business Registration Proof addressed to The Visa Officer, Embassy of Czech Republic, New Delhi. Otherwise NOC from Employer in Original along with last 6 months updated Pay slips.
Overseas Travel Medical Insurance valid across all Schengen countries .The insurance should cover the Applicant for all risks like accidents, illness, and medical-emergency evacuation for an amount of at least 30,000 Euros or equivalent. The policy should clearly specify the period of validity, covering the entire duration of the trip including the date of arrival & departure.
Return air ticket.
Hotel Reservation as per the Stay shown in Air Ticket (GTA Bookings will not be accepted)
Day to day tour Itinerary.
Evidence of source of Financial support such as :

Personal Bank statement for last six months in original duly stamped and signed + Copy of updated Personal ITR for last 3 years.

Company’s Last 6 months updated Personal in original with Bank seal and Copy of Last 3 years IT Returns.
NOC from School in Original if Kids are studying in Original.
Two Copies of first & last page of the passport to be submitted along with copies of previous Schengen/ US/ UK and Canada Visas.
Applicant could be asked for additional documents or may be called for an Interview if desired by The Embassy.

Note : We can submit the Application to Embassy as per the Appointment by the Embassy.

In case of Business, above mentioned Documents are needed along with the Copy of Invitation letter on Invitee Company’s letter head.
